Why do people assume publishing an ebook is any different than publishing a print book? Ebooks are not the same as self-publishing.

Publishers will consider books that were published by other publishers, but if it was self-published first, then you'll have to really impress them and show high sales for them to accept it. Self-publishing a book first shows as amateurish and publishers will laugh at you if you call it a publishing credit.

Conversely to @PortableWorld's advice for a reader to suggest things that are boring to be cut, have a reader read it to see where they'd like it to be longer or to add more information. Comments like "Oh, I wish you had gone into more detail about Jake's mother's arrest" could give you the extra length you need.

Italics is the common way of stressing things in Fiction writing instead of Caps. Readers tend to think that caps look amateurish and equate caps with shouting. Italics is a subtler way of adding emphasis.
